December 27, 2013 - Friday - Kimball, West Virgina - Freight train derailment
http://www.bdtonline.com/breakingnews/x1186896779/Train-derails-in-McDowell-County-tanker-car-crashes-into-Elkhorn-River

Seventeen cars from a train have derailed this morning in McDowell County, including a tanker car that is in the Elkhorn River, emergency officials said. The derailment occurred near Kimball between the Vivian and Landgraff communities, according to John Sidote, a public information officer with the McDowell County 911 Emergency Communications Center. At least 17 cars are involved in the derailment, including two tankers. One of the two tankers is in the Elkhorn Creek. No evacuations have been ordered at this time, and U.S. Route 52 has not been closed at this time. Sidote said officials do not know what type of liquid the tanker car is carrying. Regional hazardous material crews have been contacted, and are responding, including a team from Charleston, a Norfolk Southern team and a hazmat crew with the Welch Fire Department, along with the Kimball Volunteer Fire Department. The tanker car in the Elkhorn River is believed to have a leak. Robin Chapman, a spokesman for Norfolk Southern, said the tanker leaking fluid into the Elkhorn Creek contained asphalt. Chapman said NS has hazmat teams on the way.

December 10, 2013 - Tuesday - Bakersfield, California Freight train derailment
http://www.bakersfieldcalifornian.com/local/x429883529/Freight-train-derails-west-of-Tehachapi

Seven cars of a Burlington Northern Santa Fe freight train derailed about four miles west of Tehachapi Tuesday morning, sending one down an embankment and leaving mangled track along the side of the mountain. Special Agent Lee Henning of Union Pacific said the derailment happened between 6 and 6:15 a.m. along tracks hugging the side of the mountain just south of the Highway 58 through the Tehachapi Pass. The area of the derailment was about eight miles east of the Tehachapi Loop in the Golden Hills area.
Henning said about 200 feet of track was damaged in the derailment. An estimated 70 feet of track ended up below the railbed. Kern County Sheriff and Fire Department personnel responded to the derailment but left the scene by about 7:45 a.m. There were no injuries. The cause of the derailment is not yet known. Henning said no hazardous materials were on the train. BNSF spokeswoman Lena Kent said the train contained “freight of all kinds.” She said she couldn’t be more specific, but a sheriff’s spokesman said the derailed cars carried containers filled with auto parts.

The Tehachapi Pass rail corridor has long experienced congestion and delays, with state officials classifying a rail upgrade as “critical.” The Tehachapi Rail Improvement Project would "double-track" two segments — a total of 1.38 miles — of an existing route linking the Central Valley to points east. Kern Council of Governments Executive Director Ahron Hakimi said the association supports safety improvements on BNSF and Union Pacific lines and has long supported increasing double-tracking on both sides of Bakersfield. He said increasing capacity through double-tracks is likely to reduce accidents in areas that just have the one track passing through.

A mile-long train carrying crude oil has derailed and exploded, triggering a "giant fireball" after colliding with another train in North Dakota on Monday. The freight train carrying crude oil hit another train hauling grain that had derailed on Monday afternoon, causing an explosion and sending flames shooting more than 100 feet into the air. No injuries were immediately reported. Authorities said local emergency crews responding to reports of a derailment discovered the oil train burning, with up to 10 cars fully engulfed.
“There was an explosion, where a car let loose and there was a giant fireball, hundreds of feet in the air,” said Assistant Chief Gary Lorenz of the City of Fargo Fire Department, who was in touch with a crew on the scene. The collision occurred at a street intersection just before 2:20 p.m., when the westbound BNSF Railway train carrying grain derailed and was then hit by the eastbound train carrying oil, Cecily Fong, a public information officer with North Dakota Emergency Services, told NBCChicago.com. Both trains were owned by BNSF, she said.

December 27, 2013 - Saturday - Bayard, New Mexico Iron Ore Freight train derailment
http://www.scsun-news.com/silver_city-news/ci_24630639

Three train operators were killed Saturday when a locomotive for Southwestern Railroad derailed around noon and fell 40 to 50 feet into an arroyo off State Highway 356 at mile post three, three miles from Bayard. The cause of the accident was unknown as of late Saturday afternoon. The train derailment caused a diesel fuel leak, and members of the Silver City Fire Department worked to contain the leak and a hazmat operation was underway late Saturday to clean up the spill. The locomotive was pulling eight cars south to Santa Rita when it fell off the track. The eight supply cars continued to run along the track until they reached a curve in the tracks and then derailed. The Federal Railroad Administration has been contacted and will investigate the cause of the accident, Sgt. Sosa said. The accident occurred on Freeport-McMoRan Copper and Gold property. Freeport-McMoRan Copper and Gold owns and operates Chino Mine.

3 Amtrak Trains with hundreds of Passengers stranded since Monday due to drifting snow
_http://gma.yahoo.com/3-amtrak-trains-hundreds-passengers-stranded-since-monday-124000233--abc-news-topstories.html

About 500 passengers aboard three Amtrak trains were stranded overnight in a remote part of northern Illinois because of blowing and drifting snow, Amtrak officials said today.

The trains were halted late Monday near Mendota, about 80 miles west of Chicago. The passengers were aboard the Southwest Chief from Los Angeles, the Illinois Zephyr from Quincy, Ill., and the California Zephyr from the San Francisco Bay area, Amtrak spokesman Marc Magliari told The Associated Press.

The trains became stuck around 4:15 p.m. ET Monday after they hit a 12-foot snow drift that paralyzed the engines, passenger Bryan Plummer told ABC News today by cellphone.

Frigid 'Polar Vortex' Cripples US for Second Straight Day

Amtrak officials say they are working to get the passengers off the trains safely.

"We've completed unloading the first train," spokesman Magliari told ABC News this morning. "We're now moving to unload the second. And all those passengers from those trains who spent the night overnight in Bureau County will be on chartered buses coming here to Chicago this morning."

Spokesman Schultz says snow and ice on the tracks are the main concerns and there's no word on when they will move.

■CN Rail says 16 cars derailed, 8 carrying dangerous goods and one locomotive
■First responders unable to approach the fire amid safety concerns
■Hazardous material responders expected to arrive overnight
■No injuries reported

A CN freight train carrying dangerous goods has derailed and caught fire in northwest New Brunswick, not far from the U.S. border. Jim Feeny, director of public and government affairs for CN Rail, said 16 cars are believed to have derailed, including four carrying propane and four carrying crude oil. The train derailment happened just after 7 p.m. AT about five kilometres outside of Plaster Rock in Wapske, N.B. Feeny said the train was coming from central Canada and heading to Moncton. The derailed cars include one at the front, and 15 at the back. The 15 include the hazardous goods and a locomotive. A large red glow could be seen a significant distance away as the fire burned. First responders were forced to stay a safe distance back. A two-kilometre radius around the fire has been evacuated. Feeny said that includes 45 households.
There's no time frame for when crews will be able to move in. CN hazardous material experts were flying in from Moncton, Montreal and Toronto overnight. Feeny said senior managers were also en route to direct the operation. He said the safety of the public is the top priority. Feeny said it's too early to determine what caused the derailment.

It's been more than 24 hours since a Canadian National train traveling from Jackson, Mississippi to Mobile, Alabama derailed in New Augusta. Since 9 a.m. Friday, emergency officials and agencies have worked non-stop to get the entire area cleaned-up.

......“It’s around 18 derailed cars, there's a lot of cars full of crude oil over there, there's cars that have leaked, there's crude oil in the ditch right here that has to be addressed,” said Mississippi Department of Environmental Quality Environmental Scientist Ernie Shirley. “There's a rain even possibly coming tomorrow that we have to be prepared for, there's a lot of work, there's air monitoring going on, so there's a lot than besides just opening the track and that type of stuff.” ..........

......Many of the families who were evacuated Friday are now back in their homes......

..... “It's going to happen in probably a week to get that cleaned up,” said Shirley.

While cleanup crews work on offloading the methanol and cleaning the crude oil, a cause to the accident is still being determined.

“They'll try to figure out exactly what happened, but right now I don't think there's going to be a good answer for that,” said Carr. ........

Another train carrying fuel and chemicals has derailed in the latest in a series of similar accidents.
This time, a Canadian National Railway (CNI) train ran off the track in Southern Mississippi on Friday.
The train was transporting fuel oil, liquid fertilizer and methanol, and while some of the cargo leaked, there were no explosions. The incidents have raised doubts about the safety of moving combustible goods by train.
The accident in Mississippi came on the day that the State Department released a report that was favorable to TransCanada's (TRP) Keystone XL pipeline.

02/08/2014 - Falling rock derails train in French Alps, two dead

Reuters) - A huge falling rock derailed a train in the French Alps on Saturday, killing two people and partly pushing one of its carriages over the edge of a mountain slope, officials said.

Nine people were injured, one of them seriously, in the accident, which occurred on Saturday morning on a train line popular with tourists between the villages of Saint-Benoit and Annot in southeastern France, the officials said.

"An enormous rock broke off from the mountain and hit the side of the train with extreme force," Charbel Aboud, deputy prefect for the region, told a news conference, adding it may have weighed as much as 20 tonnes.

The two dead were a French woman from the region and a Russian woman who was travelling with her husband, he said.

Officials said it was the first such accident known on the line and it was unclear why such a large rock fell.

"It is too early to establish what were the factors, notably the weather conditions, that may have led to this boulder to break off," Transport Minister Frederic Cuvillier told reporters.

Southeastern France has experienced extremely wet weather in the past month, leading to flooding in the coastal Var region and mirroring floods in the northwest.

Netting is used along the line to prevent rockfalls, local officials added.

Seven people were killed last year when a mainline train derailed south of Paris in an accident that has been attributed to a fault on the track.

http://www.sott.net/article/269517-Update-Helicopter-crashed-into-Glasgow-pub-leaving-8-dead-14-injured-Tragic-event-marked-black-St-Andrews-Day-for-Scotland

There is news on this helicopter business:

A police helicopter which crashed into a busy Glasgow pub killing 10 people suffered a double engine failure, air accident investigators have found.

The Air Accident Investigation Branch (AAIB) said both engines "flamed out" but its report does not pinpoint the cause. Some commentators have suggested a problem with the fuel supply.

The Eurocopter EC 135 came down on the Clutha Bar on Friday 29 November.

All three people on board and seven others in the pub were killed.

In its report, the AAIB said that its investigation into the crash was still ongoing.

Investigators said they were still trying to establish why both engines "flamed out" when there was 76kg of fuel remaining.

Examination of the engines showed "no evidence of foreign object damage or intake or exhaust blockage in either engine".

Also, the report said no faults were found with the transmission or rotorsystem, there was no evidence of structural failure or in-flight fire and no evidence of damage caused by bird strike or a foreign object hitting the aircraft in flight.

An examination of the fuel pump switches at the accident site showed that the No 1 and No 2 prime pump switches were set to the 'on' position but the fore and aft transfer pump switches were set to the 'off' position.

No CCTV recordings had been obtained which captured the end of the flight and the recorded radio transmissions did not contain any reference by the crew to difficulties with the aircraft.

The AAIB is also investigating why there was no Mayday call and why the aircraft was not able to make a controlled landing.

An initial report on the crash, which was released on 9 December, said there was "no evidence" of major engine or gearbox failure.

Recorded data indicates that, in the later stages of the flight, the right engine flamed out, and shortly after the left engine flamed out”

Although the aircraft did not have a black box data recorder, the AAIB investigation has been able to piece together elements of its last flight.

It established that the Police Scotland helicopter took off at 20:45 with 400kg of fuel on board.

It stayed over an area of Glasgow's south side for about 30 minutes before making a short 10 minute foray over Dalkeith in Midlothian, some some 38 nautical miles away.

The helicopter was granted permission to re-enter Glasgow air space at 22:18 but radar contact was lost at 22:22.

The latest AAIB report states: "Recorded data indicates that, in the later stages of the flight, the right engine flamed out, and shortly after the left engine flamed out."

The report states that the rotor blades and tail rotor were not rotating at the time of impact.

However, it does not offer an explanation as to why both engines failed. It notes that there was no blockage on the fuel lines, both fuel pumps were in working condition and there was no leakage from the fuel tanks either before or after the accident occurred.

The report concluded: "The AAIB investigation continues to examine all the operational aspects of this accident and to conduct a detailed engineering investigation.

"In particular, the investigation will seek to determine why a situation arose that led to both the helicopter's engines flaming out when 76 kg of fuel remained in the fuel tank group, why no emergency radio transmission was received from the pilot and why, following the double engine failure, an autorotative descent and flare recovery was not achieved.

"The AAIB will report any significant developments as the investigation progresses."

Following publication of the report, Bond Air Services, which operated the helicopter for Police Scotland, said the development was the "another step" in "a long and complex investigation".

"While the investigation continues, we would urge against speculation," the company said.

May 02, 2014 - Friday - New York City - Passenger train derailment
_http://news.yahoo.com/york-city-subway-train-derails-4-hospitalized-221848957.html

NEW YORK (AP) — A subway train carrying 1,000 passengers shook through a tunnel, tilted and derailed on Friday, injuring more than a dozen people and frightening scores of others with sparks, smoke and sudden darkness. Four people suffered serious injuries and were hospitalized, firefighters said. Some complained of chest pains. Fifteen others were treated at the scene. The express F train was heading for Manhattan and Brooklyn when six of its eight cars derailed at 10:40 a.m. about 1,200 feet south of the 65th Street station in the Woodside section of Queens. Dozens of firefighters, police officers and paramedics converged. They used ladders to help passengers descend from the train to track level and guided them along the track to a sidewalk opening. The derailment happened about 30 feet below street level. Power was cut to the third rail to aid the rescue. Deputy Assistant Fire Chief James Leonard said the middle six cars of the eight-car train derailed but remained upright. The cause of the accident, which damaged the express tracks, was unclear. There was no switch in the area, and the tracks were no more than 20 to 30 years old, Metropolitan Transportation Authority chairman Tom Prendergast said. The train's operator and conductor will be tested for drugs and alcohol, he said.

April 30, 2014 - Wednesday - Greenville, North Carolina Freight train derailment
_http://www.wnct.com/story/25386204/greenville-police-investigating-train-derailment

GREENVILLE, N.C. - Greenville Fire-Rescue are investigating an early Wednesday morning train derailment parallel to 14th Street between Charles Blvd. and Evans Street. Melanie Cost, with CSX Corporate Communications, says five cars with a local train out of Greenville carrying mixed CSX freight derailed Wednesday morning. The call came into CSX around 4:15 a.m., people living in the area say the train derailed around 3:30 a.m. There were no reported injuries. Cost says the derailed cars carried ammonium polyphosphate, an agricultural product that is used for fertilizer, which is non-hazardous material. Two of the five train cars leaked the material, which has since been contained. CSX deployed trained response professionals to the scene, and additional environmental experts responded to the scene to asses the situation and determine the best course of action.

April 30, 2014 - Wednesday - City of Lynchburg, Virginia Crude Oil train derailment
_http://news.msn.com/us/csx-oil-train-derails-in-virginia-leaks-into-river

In its second oil-train accident this year, CSX said 15 cars of a train traveling from Chicago to Virginia derailed at 2:30 p.m. EDT. Fire that erupted on three of the cars was extinguished several hours later, the company said. The three cars, each weighing 100 tons, fell down an embankment into the river and were still leaking oil on Wednesday evening, city mayor Michael Gillette told Reuters. There were no injuries, and the nearly 350 residents who were evacuated earlier in the day were allowed to return to their homes, he said. This is the sixth fiery derailment to occur in North America since a runaway train in Lac-Megantic, Quebec, derailed and exploded, killing 47 people last July. Another CSX train carrying crude oil derailed in Philadelphia in January, nearly toppling over a bridge. The latest incident, a short distance from office buildings in the city of 77,000, brought more calls from environmentalists for stricter regulations on shipping crude oil by rail. Containing the oil spill was the city's biggest concern as cleanup efforts began, Gillette added. The river flows into Chesapeake Bay. CSX was sending a crew to clean up the wreckage on Wednesday evening, and city officials expected the company to remove the derailed tank cars by the end of business on Thursday.
