So the bottom line is that it is probably something in my diet? This makes sense as it does tend to flare up if I've ingested gluten recently. Still at the "sort of" doing it stage of the diet.

Sure, I'll give a general run down.
Every morning I will have bacon and eggs fried in either olive oil, ghee, or a combination of the two. Depending on how hungry I am I will have up to five rashers, but normally only have two eggs. The eggs have never given me any bother. After or before this I will usual have one or two cups of coffee, which I am working on getting off. I take it black with no sugar, and a recently started taking it occasionally with honey. Sometimes after the main part of breakfast I will have a small bar of dark chocolate if I am not quiet full.
Lunch I find tricky, and it can vary from a mix of apples, bananas and almonds or generic meat stews (cubed steak and or mince with tinned tomatoes and tomato puree with a mix of veg) which I normally have with a side of mashed sweet potato. It is normally the fruit option. Most of the time I will have chocolate in the middle of the day.
For my evening meal I tend to either have plain meat (chicken breast, steak, etc) with a mix of veg on the side or variations on the stews mentioned above. An example would be a chili that I have memorized that has capsicum, chopped tomatoes, chili powder, cumin, Chinese spice mix, thyme, dark chocolate, Tabasco, garlic and onions.
I brush with a non-fluoride toothpaste, and the water in my area is not fluoridated. Apart from coffee all I drink is tap water. I also smoke additive free tobacco. I am trying to stop drinking but haven't, and this is the main source of gluten I get because I tend to drink beer. I will drink about once every two weeks. The noises are tied to this also.
If I am really tired on a morning I will smoke and drink coffee before I have eaten.
Don't use any aluminium containing products such as non-stick or deodorant, and use shampoo about twice a week.
I have recently started taking multivitamins and cod liver oil.
That is basically my diet.
Something I left out of the original post is that sustained EE makes the feelings stop, and, if I keep up with it for a long time, it begins to feel like a kind of weight has been lifted from the area and that the area is kind of damaged but healing. If I get flaky with drinking and EE the cracking and popping comes back.
