Well the waiver could be standard procedure for health care workers now. When I worked in the field I could see the possibility coming that we would, one day, be put in the position of 'get the shot or leave the job' kinda thing and I really didn't want to be in that position. But since your sister didn't loose her job over not signing the waiver, it also could be a way for management to see which employees will follow authority blindly. Glad you sister opted out of the shot. Smart move on her part.
