It's simply the "consequence" of the test - each answer is attributed with points, from 1 to 5 going from left to right.
When trying to trick it, i.e. answering only on one question and submitting, the result is:

NaN/50

I got 31/50 as well. I noticed that I would answer some of the questions (1, 6, 10) differently depending on the amount of stress I'm experiencing, with increased stress correlating with more withdrawal, less availability for others, and an ultimately higher covert narcissism score. So that definitely gives me something specific to work on.

I think the test scoring is buggy. I tried it one time where my score came back as "NaN" and that's an error typically returned by Javascript when it can't find the value of a variable - in this case it should have been a number. So I had to redo the test answers.

On the last question, something along the lines of, 'being often annoyed when other people ask me to take my time to sympathize over their problems,' I hesitated between selecting neutral or uncharacteristic and chose uncharacteristic-wasn't sure I agreed with 'often'. So, could have had a higher score by a couple points maybe?

Interesting that yesterday, before seeing this thread, I decided to randomly skim through the book 'The Narcissistic Family'. I've read the book years ago, think it's probably about time I read it again.

As other mentioned, in my case there were also a bunch of questions where I chose "neutral" because the way I would react would depend not only on circumstances, but sometimes even a particular period of the month. :-[ Kind of tricky, because in some cases the thought process didn't really go away completely, with time it just got easier to redirect it to something much more useful.

With the same circumstances in mind, I got 35/50. Full moon, winter time, overall tiredness and/or stress, hunger - many factors may influence my choices at some points. Being aware of these influences helps counteracting them in some cases, but not in other. When I'm hungry and sleepy, it's not a good time to challenge my "awesomeness". :-[ :D

Still a lot of Work to do, I guess.
